i find going as slow as i can at the beginning helps. sell the disease you get at the beginning, level 1 cold heat and moisture. then be patient  save up the points to hit them as hard as possible once everyone is infected

I got Madagascar by spending a lot of points on highly infectious symptoms so that it would spread to other countries too fast for them to counter it by closing the shipyard.
Infected every country within a week but then had to wait a while to develop more lethal symptoms. Got all of the hospitals closed before they could finish the vaccine so it was only a matter of time then.
